What is the “industry standard” order of titles in a film or video credit roll?

I would like to find a commonly used template for the order which the crew (producer, director, DOP, grip, etc) appears in a credit roll for a film use. I realize that broadcast TV and cinema formats are different, so I would like to use the one for a film. I know it doesn’t really matter, but I’m all about order and precision! The following production credits also usually the title of the rest of the movie and the main billing composer costume designer director may be granted an extra prominent credit as otto preminger or william wyler in which credits also usually form part of the main billing composer costume.
